Pellagra emerged as a devastating epidemic condition in eighteenth-century Europe, appearing most prominently in populations who had adopted maize as a dietary staple following its introduction from the Americas. For well over a century, physicians and public health authorities debated its origin, variously attributing it to infectious causes, toxic substances, and social deprivation before a nutritional explanation was finally established. Historical Narrative
The earliest clear clinical descriptions of pellagra as a distinct condition appeared in eighteenth-century Spain, where the physician Gaspar Casal observed and documented the illness among impoverished rural populations in the Asturias region. Casal, who encountered the condition around 1735 and wrote about it in manuscript form before his death in 1759, described the characteristic skin changes affecting sun-exposed areas, as well as the debilitation he observed in affected individuals, and noted a strong association with poverty and a diet heavily dependent on maize. His posthumously published account, appearing in 1762, named the condition mal de la rosa after the reddened appearance of the skin, and it represented the first substantive medical documentation of the disease in the European literature.
Around the same period, the condition was being independently observed in northern Italy, where it had become widespread among peasant populations in Lombardy and the Veneto. The Italian physician Francesco Frapoli, writing in 1771, applied the term pellagra, derived from Italian words for rough or pelled skin, and this nomenclature ultimately prevailed in international medical usage. Italian physicians of the eighteenth and nineteenth centuries became the leading investigators of the condition, and the Italian agricultural regions where it was endemic served as an ongoing site of medical inquiry.
Through the nineteenth century, debate about pellagra's causation was vigorous and often contentious. Many Italian and European investigators favored the theory that spoiled or fungus-contaminated maize was the causative agent, a hypothesis that became known as the zeist position and attracted influential proponents including the Italian physician Cesare Lombroso, better known for his later work in criminal anthropology. Lombroso championed the idea that a toxic mold present in deteriorated corn was responsible for the disease, a view that aligned with contemporaneous germ theory enthusiasm and seemed to explain geographic and seasonal patterns in the illness.
Other investigators, observing that the disease tracked closely with social conditions and poverty rather than with maize consumption alone, proposed that nutritional inadequacy or poor sanitation might play important roles. The debate remained unresolved through most of the nineteenth century and into the early twentieth, when pellagra emerged as a major public health crisis in the American South, where poor sharecropper communities subsisting on diets of corn meal, salt pork, and molasses were experiencing epidemic rates of illness and death.
The resolution of the pellagra controversy came largely through the work of Joseph Goldberger, a physician employed by the United States Public Health Service, whose investigations beginning in 1914 systematically dismantled the infectious disease hypothesis. Goldberger conducted a series of carefully designed studies in Southern institutions and in prison populations, demonstrating that dietary intervention could prevent and reverse the condition and that it could not be transmitted between individuals. His work, which included the notorious filth parties he organized to demonstrate non-infectivity, established pellagra as a deficiency disease of diet and shifted attention to identifying the specific nutritional factor involved.
The biochemical identity of the missing nutrient was established in 1937 by Conrad Elvehjem and colleagues at the University of Wisconsin, who demonstrated that nicotinic acid, later called niacin, cured a related condition in dogs and subsequently proved effective in human pellagra. This discovery retroactively explained why populations in Mesoamerica, where maize had originated and was prepared through a lime-soaking process known as nixtamalization that renders niacin biologically available, had not historically suffered from the disease.
