Pernicious anemia was regarded for much of the nineteenth century as an invariably fatal condition, a mysterious and progressive destruction of the blood that resisted all available treatments and carried a grim prognosis. Physicians debated for decades whether the disease originated in the blood itself, in the stomach, in the nervous system, or in some toxic agent produced within the body. The eventual discovery that a dietary factor could reverse the condition overturned longstanding assumptions and became one of the most celebrated medical achievements of the twentieth century.

Historical Narrative

The first clinically detailed description of the condition now called pernicious anemia was published by the British physician Thomas Addison in 1849, in a brief but precise account of patients who developed a profound and progressive anemia accompanied by general weakness and a waxy pallor of the skin. Addison distinguished this form of anemia from blood loss and from anemia caused by obvious organic disease, characterizing it as idiopathic and ultimately fatal. His account attracted limited attention at the time, and the condition was subsequently referred to in some medical literature as Addison's anemia in his honor.

The German physician Michael Anton Biermer provided a more extensive clinical description in 1872 and introduced the term 'pernicious anemia,' which became the standard designation in medical literature for decades. Biermer's detailed reports emphasized the progressive nature of the blood changes and the characteristic large red blood cells that distinguished this condition from other anemias, though the cellular understanding of blood was still developing during his era. His work stimulated considerable European interest in the condition, and clinicians across Germany, France, and Britain began identifying and reporting cases.

Throughout the late nineteenth century, competing theories about the cause of pernicious anemia proliferated. Some investigators proposed that a toxin produced by intestinal bacteria was destroying the blood-forming elements of the bone marrow. Others focused on observed changes in the stomach, noting that many patients with the condition appeared to lack normal gastric secretion, and hypothesized that the stomach's failure to produce some essential substance was responsible for the blood disorder. Neurological changes observed in some patients added further complexity, and debates continued about whether these represented a primary feature of the disease or a secondary complication.

The transformation in understanding came dramatically in 1926, when the American physicians George Minot and William Murphy published their landmark paper reporting that feeding large quantities of raw liver to patients with pernicious anemia produced remarkable and rapid improvement. Their clinical trial, involving forty-five patients, demonstrated beyond reasonable doubt that something present in liver tissue could reverse a condition that had previously been considered universally fatal. The work of William Bosworth Castle, who conducted ingenious experiments in the late 1920s and early 1930s, elaborated on these findings by proposing that normal stomach lining produced an 'intrinsic factor' that interacted with an 'extrinsic factor' in dietary protein to enable proper blood formation. Castle's experiments involved recovering gastric juice from his own stomach after ingesting beef muscle and demonstrating that this material could benefit patients in ways that neither beef muscle nor gastric juice alone could achieve.

Minot and Murphy, along with the physiologist George Whipple whose prior experiments on liver feeding in anemic dogs had inspired their work, were awarded the Nobel Prize in Physiology or Medicine in 1934. The subsequent isolation of the active principle in liver by researchers including Mary Shaw Shorb and the eventual crystallization of vitamin B12 by Karl Folkers and his colleagues in 1948 closed a chapter that had begun with Addison's puzzled clinical observations nearly a century earlier.
