Medin's student Ivan Wickman conducted exhaustive epidemiological investigations during the devastating Swedish epidemic of 1905, tracing patterns of spread between individuals and households. His work strongly supported the theory that poliomyelitis was a communicable disease, and he emphasized that mild or inapparent cases might serve as sources of transmission — a radical and controversial idea at the time. The infectious nature was confirmed in 1908 when Austrian scientists Karl Landsteiner and Erwin Popper successfully transmitted the disease to monkeys using filtered spinal cord material from a human case, establishing that a filterable agent — later understood to be a virus — was responsible.

American researcher Simon Flexner at the Rockefeller Institute subsequently spent years studying the disease in primates, though his conclusions about the virus's route of entry through the nasal passages temporarily misdirected research efforts. Enormous epidemics struck American cities repeatedly through the early twentieth century, with the 1916 New York outbreak killing thousands and paralyzing thousands more, generating widespread public fear and quarantine measures of variable effectiveness.

The high-profile paralysis of future United States President Franklin D. Roosevelt in 1921 transformed public engagement with poliomyelitis research. The iron lung, developed in the late 1920s by Philip Drinker and Louis Shaw, became an iconic fixture of mid-century hospital wards, mechanically assisting breathing in patients whose respiratory muscles had been paralyzed.

John Enders, Thomas Weller, and Frederick Robbins earned the Nobel Prize in 1954 for their 1949 breakthrough in cultivating poliovirus in non-neural tissue, making vaccine development practically feasible. Jonas Salk's inactivated vaccine, tested in one of the largest clinical trials in history during 1954, was announced effective in 1955, setting off jubilant celebrations. Albert Sabin subsequently developed an oral live-attenuated vaccine that became the foundation of global eradication campaigns in the latter half of the twentieth century.
