The earliest hints of what would eventually be categorized as porphyria appeared in ancient medical texts, though healers of the time had no framework to recognize a unified disorder. Hippocratic writers documented cases of patients passing dark or reddish urine accompanied by various systemic complaints, attributing such presentations to imbalances among the four humors. Urine discoloration was generally interpreted through a humoral lens, with dark or port-wine-colored urine assigned to an excess of black bile or a corruption of the blood. Medieval physicians, following Galenic traditions, similarly interpreted unusual urine colors as signs of internal putrefaction or spiritual imbalance, and some cases recorded in Arabic medical encyclopedias may have described what later generations would recognize as porphyric episodes.

The word porphyria itself was derived from the Greek porphyra, meaning purple, and the formal naming of the condition came only in the early twentieth century. The Swiss physician Felix Hans Günther is credited with coining the term in 1911, when he undertook one of the first systematic classifications of the various forms of the disorder based on the accumulation of pigmented compounds in urine and other tissues. His work built upon a foundation laid in the latter half of the nineteenth century, when chemists and clinicians began identifying the specific pigments responsible for the characteristic discoloration.

Johannes Müller and later investigators in German-speaking academic medicine contributed significantly to the chemical understanding of bile pigments and heme-related compounds during the mid-nineteenth century. The Scottish physician John Macgregor described a family with recurrent attacks of dark urine in the 1870s, and this case attracted considerable attention as physicians began to suspect a hereditary element. By the 1880s and 1890s, the Dutch physician Barend Joseph Stokvis had described cases of acute urine discoloration following exposure to the sedative sulfonal, which represented one of the earliest recognitions that certain chemical agents could provoke episodes in susceptible individuals.

The early twentieth century brought a more rigorous biochemical approach to the condition. Hans Fischer, the German chemist who won the Nobel Prize in Chemistry in 1930 for his work on hemin and chlorophyll, made foundational contributions to understanding porphyrin chemistry. His laboratory's painstaking elucidation of porphyrin structures provided the molecular vocabulary that physicians and researchers would eventually use to distinguish different forms of the condition.

Perhaps the most culturally resonant chapter in the history of porphyria research came in 1968, when the British physician Ida Macalpine and her son Richard Hunter published a paper in the British Medical Journal arguing that King George III of Great Britain had suffered from porphyria. This retrospective diagnosis, based on historical accounts of his episodic illnesses, his reportedly blue-tinged urine, and his behavioral disturbances, captured enormous public attention and prompted widespread popular interest in the disorder. Subsequent historians and medical researchers debated this hypothesis vigorously for decades. The George III controversy also inspired further retrospective investigations into other historical figures, including members of the royal houses of Stuart and Hanover, weaving porphyria into a broader narrative of European royal history.
