The next major turning point came in 1999, when researchers Ruthann Schanen and Huda Zoghbi, building on years of genetic investigation, identified mutations in the MECP2 gene on the X chromosome as the underlying cause of the vast majority of classic Rett syndrome cases. Zoghbi's laboratory had been pursuing the genetic basis of the condition throughout the 1990s, and this discovery fundamentally transformed scientific understanding of the syndrome from a condition of unknown origin into one with a defined molecular basis. The identification of the MECP2 gene also explained why the syndrome appeared almost exclusively in females, given the lethality of the mutation in most males who lacked a second X chromosome to partially compensate.
This genetic breakthrough opened an entirely new chapter in Rett syndrome research, shifting the focus from clinical description toward understanding the molecular mechanisms by which MECP2 dysfunction affected neurological development.
