In 1851, Bilharz identified the causative worm during postmortem examinations, describing the organism he found in the portal blood vessels of Egyptian patients. He communicated his findings in letters to his mentor Carl von Siebold, and the parasite was subsequently named Distomum haematobium in early publications, later reclassified as Schistosoma haematobium in honor of Bilharz, whose name also gave rise to the disease's alternative designation, bilharzia.

Following Bilharz's identification of the parasite, subsequent decades brought further discoveries that complicated the picture. Patrick Manson, the Scottish physician whose work on mosquito-borne disease transmission earned him the title 'father of tropical medicine,' contributed significantly to the broader understanding of parasitic diseases and provided an intellectual framework within which schistosomiasis research advanced. Arthur Looss, a German parasitologist working in Egypt in the late nineteenth and early twentieth centuries, made important contributions to understanding helminth life cycles that informed later schistosomiasis research.

A major conceptual breakthrough came in the early twentieth century when Japanese physician Fujiro Katsurada described a distinct species causing different clinical presentations in Asia, which became known as Schistosoma japonicum. The collaborative work of Japanese researchers including Keinosuke Miyairi and Masatsugu Suzuki in the 1910s definitively demonstrated the role of freshwater snails as intermediate hosts, solving a crucial puzzle about how humans became infected. This discovery fundamentally reoriented public health thinking about the disease, directing attention toward the aquatic environments where transmission occurred rather than solely toward the afflicted individuals.
