Scurvy afflicted human populations for millennia and became a particularly devastating disease among sailors during the age of maritime exploration, when long ocean voyages made fresh provisions impossible to sustain. Physicians and naval surgeons for centuries debated its causes, proposing theories ranging from putrid air and moral degeneracy to dietary imbalance, before experimental observations in the eighteenth century began to point toward the preventive power of certain foods. The eventual scientific explanation for scurvy arrived only in the twentieth century with the isolation and identification of vitamin C.

Historical Narrative

Written accounts recognizable as descriptions of scurvy appeared in ancient Egyptian texts and in the works of Greek and Roman authors. Hippocratic writings contained passages describing a condition marked by gum deterioration and weakness that historians have interpreted as consistent with scurvy, and Roman military records described similar afflictions among troops on campaign. However, these ancient accounts did not cohere into a sustained medical tradition of understanding the disease, and scurvy was not systematically investigated as a distinct clinical entity until the early modern period.

The age of oceanic exploration beginning in the late fifteenth century transformed scurvy from a periodic affliction into a catastrophic public health crisis. Vasco da Gama's voyage to India in 1497 and 1498 reportedly cost the lives of a significant portion of his crew to a disease that contemporaries struggled to name and explain. The expedition of Magellan and Elcano circumnavigating the globe between 1519 and 1522 documented terrible mortality from a similar condition. European naval surgeons and expedition physicians of the sixteenth and seventeenth centuries proposed a wide range of explanatory frameworks, including corrupt sea air, excessive salt consumption, the moral failings of sailors, and various humoral imbalances. Many of these physicians believed that hard physical labor or the emotional despair of long voyages contributed to the onset of the disease.

A crucial episode in the empirical history of scurvy occurred during Jacques Cartier's expeditions to Canada in the 1530s, when indigenous peoples reportedly showed the ailing French sailors how to prepare a brew from the bark and needles of a local tree, an intervention that produced dramatic recoveries. This account, preserved in Cartier's journals, circulated in European medical and geographical literature for generations but did not produce a sustained theoretical understanding of why such a preparation might be effective.

The most celebrated figure in the history of scurvy's scientific investigation was James Lind, a Scottish naval surgeon who conducted what has often been described as one of the earliest controlled clinical experiments in medical history. In 1747, while serving aboard HMS Salisbury, Lind divided twelve sailors suffering from scurvy into six pairs and provided each pair with a different supplementary treatment while keeping their base diet constant. The pair who received oranges and lemons recovered most rapidly. Lind published his findings in A Treatise of the Scurvy in 1753, arguing forcefully for the efficacy of citrus fruit, though his theoretical explanation for why citrus worked remained tied to the humoral and acidic theories of his era rather than to any concept of a dietary deficiency.

Despite Lind's findings, the British Navy was slow to adopt systematic citrus supplementation. It was not until 1795, under the influence of Gilbert Blane and the advocacy of naval physicians who had continued to document the efficacy of lemon juice, that the British Navy made lemon juice a standard provision for sailors. This policy change dramatically reduced scurvy's toll in British naval vessels.

The theoretical leap from observing that citrus prevented scurvy to understanding why required the development of nutritional science in the late nineteenth and early twentieth centuries. Axel Holst and Theodor Frølich, Norwegian researchers investigating shipboard dietary diseases, produced a scurvy-like condition in guinea pigs around 1907, establishing an animal model that later investigators exploited extensively. The isolation of the antiscorbutic factor itself was achieved in the early 1930s, primarily through the work of Albert Szent-Györgyi, who isolated hexuronic acid from adrenal glands and other tissues, and Charles Glen King, who demonstrated this substance's activity against scurvy. The compound was subsequently named ascorbic acid and identified as vitamin C, finally providing a biochemical explanation for observations that healers and physicians had recorded across more than two thousand years.
