Avicenna, the eleventh-century Persian polymath, devoted considerable attention to splenic disorders in his Canon of Medicine. Andreas Vesalius, in his landmark 1543 work De Humani Corporis Fabrica, corrected numerous Galenic anatomical errors regarding the spleen's structure, though the functional understanding of splenic disease lagged behind structural description. Physicians continued to regard the organ with a mixture of curiosity and confusion, noting its dramatic enlargement in patients suffering from the intermittent fevers rampant in marshy European lowlands.

By the eighteenth and nineteenth centuries, physicians increasingly linked massive splenic enlargement to specific febrile diseases, particularly the ague they associated with miasmatic swamp air. When Charles Laveran identified the malarial parasite in 1880 and Ronald Ross confirmed mosquito transmission in 1897 and 1898, the ancient association between recurring fever and splenic swelling finally received a biological explanation. Pathologists of the late nineteenth century, equipped with microscopes, began cataloguing the various cellular and structural changes within enlarged spleens, distinguishing infectious causes from what appeared to be primary blood disorders. Rudolf Virchow's foundational work in cellular pathology during the 1850s provided the intellectual scaffolding that allowed splenic enlargement to be studied as a manifestation of underlying cellular disease rather than humoral imbalance, marking a decisive turning point in the condition's medical history.
