Historical Narrative

Some of the earliest recorded references to misaligned eyes appear in ancient Mesopotamian omen texts, where crossed eyes were interpreted as significant signs requiring interpretation by priestly physicians. Ancient Egyptian medical papyri also contained references to eye disorders more broadly, and wall paintings occasionally depicted individuals with apparent ocular misalignment, suggesting cultural and medical awareness of the condition stretching back to at least 1500 BCE.

In ancient Greece, the Hippocratic Corpus touched on squinting eyes, attributing them in some instances to an imbalance of humors or to the influence of heredity. Aristotle discussed visual phenomena related to misaligned eyes in his writings on natural philosophy, noting the double vision that could result from pressing one eye out of alignment and drawing early attention to the relationship between eye position and binocular vision. These observations, though embedded in a pre-scientific framework, represented genuine inquiry into visual physiology.

Galen's second-century synthesis of Greek medicine incorporated discussion of the muscles of the eye, and he identified several of the extraocular muscles in his anatomical works. Galen attributed strabismus to unequal tension or weakness in these muscles, a fundamentally correct anatomical intuition even if his explanations of the underlying causes remained bound to humoral theory. His writings remained authoritative throughout the medieval period in both European and Islamic medical traditions.

Medieval physicians across the Islamic world and Europe relied heavily on Galenic and Hippocratic frameworks. Ibn Sina's Canon of Medicine addressed strabismus within its comprehensive survey of eye diseases, recommending various treatments including exercises, bandaging, and topical preparations intended to strengthen or relax the offending muscles according to humoral principles.

The Renaissance anatomists revisited ocular anatomy with fresh eyes. Andreas Vesalius provided corrected descriptions of the extraocular muscles, and subsequent anatomists refined understanding of their attachments and actions. This anatomical clarity set the stage for surgical thinking, though operative intervention on the eye remained rare and dangerous by the standards of early modern surgery.

A major turning point in the history of strabismus came in 1839, when the German surgeon Johann Friedrich Dieffenbach performed and published accounts of strabismus surgery involving the cutting of extraocular muscles, a procedure known as tenotomy. The operation spread rapidly across Europe and America, representing one of the first successful elective eye surgeries and generating enormous excitement in the ophthalmological community. However, enthusiasm quickly gave way to more measured assessment as surgeons discovered that results were variable and sometimes overcorrected.

The development of ophthalmology as a formal specialty in the nineteenth century, catalyzed by figures such as Albrecht von Graefe in Germany and William Bowman in Britain, brought increasingly rigorous clinical study to strabismus. Von Graefe made important contributions to understanding the mechanics of ocular motility and the classification of different forms of strabismus, distinguishing paralytic from concomitant forms and linking their different behaviors to distinct anatomical and neurological origins.

By the late nineteenth century, the relationship between strabismus and amblyopia was being actively studied, and researchers were developing more systematic approaches to the surgical correction of misalignment. The invention of the ophthalmoscope by Hermann von Helmholtz in 1851 further enriched examination of the eye and contributed to the broader scientific environment in which strabismus research flourished.
