Syphilis emerged into the written historical record with dramatic suddenness in Europe at the close of the fifteenth century, touching off centuries of debate about its origins that has never been fully resolved. Physicians across Europe, the Americas, and Asia struggled to understand and treat a disease that moved through populations with devastating speed and manifested in bewildering variety. The history of syphilis became deeply intertwined with the history of trade, warfare, colonialism, and the slow development of germ theory.
Historical Narrative
The question of where syphilis originated consumed medical scholars for centuries and remained genuinely contested into the modern era. One school of thought, the Columbian hypothesis, held that sailors returning from Christopher Columbus's 1493 voyage brought the disease from the Americas to Europe, pointing to the timing of the first well-documented European epidemic and to skeletal evidence from pre-Columbian American remains. A competing theory argued that syphilis had existed in the Old World long before Columbus and had simply gone unrecognized or been confused with leprosy and other skin diseases. Paleopathological evidence continued to fuel the debate through the twentieth century without producing universal consensus.
The first unambiguous European epidemic erupted among the armies of Charles VIII of France during the Italian Wars of 1494 to 1495. The disease spread with remarkable speed through troops and civilian populations alike, producing severe skin lesions, bone pain, and in many cases rapid death in its early decades of circulation. European nations promptly blamed one another for the epidemic: the French called it the Neapolitan disease, while much of Europe called it the French disease, or morbus gallicus, a name that persisted in medical literature for generations.
Girolamo Fracastoro, an Italian physician and poet, gave the disease the name by which it would eventually be universally known in his 1530 poem Syphilis sive morbus gallicus, in which a shepherd named Syphilis was cursed by the god Apollo with the disease as punishment for impiety. Fracastoro was also a pioneering thinker about contagion more broadly, arguing decades before germ theory that diseases were transmitted by tiny imperceptible particles he called seminaria, seeds of disease that could pass from person to person or through contaminated objects.
The dominant early treatment for syphilis was mercury, applied as ointments rubbed vigorously into the skin or administered as vapor in hot fumigation chambers. Physicians across Europe adopted mercury on the basis of its apparent ability to induce sweating and salivation, which were interpreted as expelling the corrupt humors responsible for the disease. The treatment was extraordinarily harsh, producing tooth loss, neurological damage, and death in many patients, giving rise to the grim contemporary saying that a night with Venus led to a lifetime with Mercury.
Guaiacum wood, imported from the Caribbean and enthusiastically promoted in the early sixteenth century, briefly rivaled mercury as the preferred treatment. Ulrich von Hutten, a German humanist who suffered from syphilis himself, wrote a celebrated account of guaiacum therapy in 1519 that spread its use across Europe. The enthusiasm for guaiacum faded as its limitations became apparent, and mercury reasserted its dominance in medical practice.
Philippe Ricord, a French physician working in the mid-nineteenth century, made foundational contributions by distinguishing syphilis clearly from gonorrhea, which had often been confused with it, and by systematically describing the progression of syphilitic infection through distinct stages based on his observations of thousands of patients in Parisian hospitals. His work established the clinical framework that subsequent researchers built upon.
The bacteriological revolution transformed the scientific understanding of syphilis when Fritz Schaudinn and Erich Hoffmann identified Treponema pallidum as the causative organism in 1905. August von Wassermann rapidly followed with the development of a serological diagnostic test in 1906. Paul Ehrlich then spent years systematically testing chemical compounds for activity against the organism, ultimately identifying arsphenamine, marketed as Salvarsan, in 1909, which became the first modern chemotherapeutic agent specifically targeted at an infectious disease.
