Among the earliest suspected references to typhoid fever appear in ancient Greek texts, where Hippocrates and his contemporaries described prolonged fevers accompanied by severe systemic illness. Scholars have long debated whether the plague of Athens in 430 BCE, meticulously chronicled by Thucydides, represented a typhoid outbreak, though this interpretation remained contested across generations of historians and physicians.

For much of the medieval and early modern period, typhoid was lumped together with typhus and other 'continued fevers' under a broad and imprecise nosological umbrella. European physicians lacked the conceptual tools to distinguish these conditions with any reliability, and mortality during epidemic periods remained catastrophic. Military campaigns were particularly susceptible, with historians later attributing significant troop losses during various conflicts — including campaigns of the Thirty Years' War and the American Civil War — largely to typhoid rather than battlefield wounds.

A decisive step toward separating typhoid from typhus came in the early nineteenth century. The French physician Pierre-Charles-Alexandre Louis conducted rigorous post-mortem studies in Paris during the 1820s and 1830s, documenting characteristic lesions in the intestinal tract, particularly in the Peyer's patches of the ileum. His meticulous numerical approach to clinical medicine, later called the 'numerical method,' helped establish typhoid as a distinct pathological entity. Simultaneously, the American physician William Wood Gerhard drew a careful clinical and anatomical distinction between typhus and typhoid in Philadelphia in 1837, work that proved influential in consolidating the separation of the two diseases.

The bacteriological era brought the most transformative advances. In 1880, the German physician Karl Joseph Eberth identified rod-shaped bacteria in the spleens and lymph nodes of patients who had died of typhoid, and these organisms were subsequently confirmed and cultured by Georg Gaffky in 1884. The bacterium, eventually named Salmonella typhi, became one of the foundational subjects of early bacteriology.

Understanding of transmission underwent a parallel revolution. The British epidemiologist William Budd, working in Bristol during the 1850s, argued forcefully that typhoid spread through contaminated water and sewage rather than through miasmatic air — a position that placed him ahead of mainstream medical opinion at the time. His findings contributed to growing pressure for sanitary reform, and as municipal water systems improved across industrializing nations in the latter nineteenth century, the incidence of typhoid in urban centers gradually declined.

Immunization research followed closely. Almroth Wright developed one of the earliest typhoid vaccines in the 1890s, conducting trials on British soldiers. The vaccine was deployed, though controversially, during the Second Boer War and later on a larger scale during the First World War, with public health authorities crediting it with dramatically reducing typhoid mortality among troops compared to earlier conflicts.

By the early twentieth century, the combination of improved sanitation, clean water infrastructure, and vaccination had transformed typhoid from an almost inevitable feature of urban life into a largely preventable condition in industrialized nations, marking one of the earliest and most celebrated triumphs of applied bacteriology and public health science.
