Uveitis, an inflammatory condition affecting the uveal tract of the eye, was recognized in various forms by ancient physicians who described redness, pain, and visual disturbance in the eye without possessing the anatomical knowledge to locate the source of inflammation precisely. For centuries, healers attributed such ocular conditions to humoral imbalances or external influences, and treatments ranged from herbal preparations to elaborate ritual remedies. The precise anatomical understanding of the uvea and the pathological concept of intraocular inflammation as a distinct disease category developed only through the cumulative work of early modern anatomists and nineteenth-century ophthalmologists.

Historical Narrative

The earliest written references to inflammatory eye diseases appear in ancient Mesopotamian medical texts, including the cuneiform tablets of the Assyrian and Babylonian medical traditions dating to approximately 2000 BCE or earlier. These texts described various ailments of the eye characterized by redness, clouding, and pain, and prescribed treatments involving plant-based preparations and incantations intended to appease divine forces believed responsible for illness. While these records cannot be precisely mapped onto modern diagnostic categories, scholars of ancient medicine have suggested that at least some described conditions may have included intraocular inflammation.

The ancient Egyptian Ebers Papyrus, dating to approximately 1550 BCE, contained extensive sections on eye diseases and their remedies, including preparations made from plant and animal materials applied directly to the eye. Egyptian healers did not conceptualize the eye in terms of its internal anatomical structures but treated ocular symptoms as unified phenomena requiring symptomatic relief.

In ancient Greece, the Hippocratic texts discussed ocular diseases within the humoral framework, attributing painful and inflamed eyes to excess of particular humors. Galen of Pergamon later expanded on ocular anatomy and disease in his treatises, distinguishing several structures of the eye and describing a condition he called 'hypopyon' — an accumulation of pus in the anterior chamber of the eye — which represented one visible manifestation of severe intraocular inflammation. Galen's anatomical descriptions, while containing significant errors, dominated ophthalmological understanding in the Islamic and European worlds for over a millennium.

The medieval Islamic medical tradition made significant contributions to ophthalmology as a specialized discipline. Hunayn ibn Ishaq, the ninth-century Nestorian Christian scholar working in Baghdad, wrote an influential treatise on the eye that was translated into Latin and circulated widely in Europe. Ibn al-Haytham, the eleventh-century Arab polymath, made foundational contributions to the understanding of optics and the anatomy of vision, and his work influenced later European thinking about the eye's structure.

The anatomical investigations of the Renaissance gradually improved the understanding of ocular structure, and by the seventeenth century, physicians were beginning to think more precisely about diseases affecting specific layers and compartments of the eye. The Dutch physician Franciscus Sylvius contributed to the understanding of inflammatory processes in the body, and his influence extended to ophthalmic conditions.

The nineteenth century marked the decisive turn toward a modern understanding of uveitis. The development of the ophthalmoscope by Hermann von Helmholtz in 1851 allowed physicians to directly visualize the interior of the living eye for the first time, fundamentally transforming the study of intraocular disease. Albrecht von Graefe, the celebrated German ophthalmologist and a towering figure of nineteenth-century eye medicine, used the ophthalmoscope extensively to study inflammatory conditions of the eye and made careful clinical distinctions between different forms of intraocular inflammation. Von Graefe's detailed clinical observations of iritis, choroiditis, and related conditions established a more precise nosology that laid the groundwork for the systematic study of uveitis.

Later in the nineteenth century, researchers including Eduard Jaeger contributed further microscopic and clinical descriptions of uveal inflammation, and the emerging field of bacteriology prompted investigations into whether infectious agents might be responsible for certain cases of intraocular inflammation, opening a new chapter in understanding the condition's diverse origins.
