Among the oldest textual references to skin depigmentation are passages found in ancient Indian medical texts, particularly the Atharva Veda, dated by scholars to approximately 1400 BCE, which contained descriptions of white skin patches that some historians of medicine have interpreted as early accounts of vitiligo. The Ebers Papyrus of ancient Egypt, dated to around 1550 BCE, similarly contained references to conditions involving loss of skin color, though scholars have long debated whether these passages described vitiligo, leprosy, tinea versicolor, or other conditions. The ancient Hebrews used the term tzaraath in biblical texts to describe various skin conditions, and the conflation of depigmentation with ritual impurity meant that individuals with lighter skin patches were often treated as socially and religiously contaminated regardless of the actual nature of their condition. In ancient Greece, physicians working within the Hippocratic tradition discussed skin discoloration but did not isolate vitiligo as a distinct entity, and the Greek terms leuke and alphos were applied broadly to several conditions involving pale patches. The Roman physician Celsus, writing in the first century CE, attempted to distinguish between different forms of skin whitening, separating what he considered more benign depigmentation from the feared presentations associated with leprosy, a distinction that proved difficult to maintain in subsequent centuries. In medieval Islamic medicine, physicians made notable contributions to dermatological classification. Avicenna, writing in the eleventh century Canon of Medicine, described multiple skin conditions with attention to differentiating features, and his encyclopedic work preserved and expanded upon earlier Greek and Roman observations. Medieval European physicians largely relied on these transmitted classical and Islamic texts, and the social fear of leprosy meant that any condition producing unusual skin appearance was treated with considerable wariness. The formal effort to distinguish vitiligo clearly from leprosy gained momentum in the eighteenth and nineteenth centuries as European dermatology developed as a medical specialty. The Viennese dermatologist Ferdinand von Hebra, a central figure in establishing dermatology on systematic pathological grounds in the nineteenth century, contributed to the more precise classification of skin diseases. His successor Moriz Kaposi further refined dermatological nosology in ways that helped separate conditions that earlier eras had grouped imprecisely. In India, the condition had long been known by the Sanskrit term shvitra, and Ayurvedic physicians had for centuries attempted treatments involving plant-based preparations, including extracts from the bakuchi plant, which contained compounds later recognized by twentieth-century researchers to belong to the psoralen family. The connection between traditional Indian plant remedies and photochemical mechanisms was investigated by Egyptian researchers in the 1940s, particularly by Abdel Monem el Mofty, who conducted studies on psoralen compounds and ultraviolet light that became foundational in the subsequent scientific literature on pigmentation. Throughout the twentieth century, researchers worked to understand the role of melanocytes in pigmentation and the processes by which those cells were lost in vitiligo, placing earlier centuries of largely descriptive and stigma-laden history on an increasingly mechanistic biological foundation.
