The earliest recognizable accounts of yellow fever appeared in Mayan records from the Yucatán Peninsula, where a disease known as 'xekik,' meaning 'blood vomit,' was described in codices dating to pre-Columbian Mesoamerica. Spanish colonizers encountered devastating epidemics in the Caribbean beginning in the 1640s, with a major outbreak recorded in Barbados in 1647 that observers described as annihilating entire communities of newly arrived Europeans. The disease earned its common English name from the jaundiced appearance that afflicted victims developed in severe cases, a coloring noted in countless colonial-era medical reports.

Throughout the 17th and 18th centuries, European physicians operating in tropical colonial settlements debated whether yellow fever arose from putrid miasmas emanating from swamps and rotting organic matter, or whether it spread from person to person through some undefined contagious principle. The miasmatic theory held tremendous authority, and colonial administrations frequently ordered the burning of goods, the firing of cannon to 'clear' infected air, and the fumigation of ships with sulfurous smoke in attempts to purify the atmosphere around afflicted ports.

Philadelphia's catastrophic 1793 epidemic, which killed roughly ten percent of the city's population, forced American physicians into open and bitter conflict over the disease's nature. Dr. Benjamin Rush, the era's most prominent American physician, championed both the miasmatic origin theory and an aggressive treatment regimen involving heroic bloodletting and massive doses of calomel, a mercury-based purgative. Rush bled patients repeatedly and purged them with extraordinary force, a practice he defended with passionate conviction even as critics such as Dr. William Currie argued that his treatments hastened death rather than preventing it. The Philadelphia epidemic became one of the most documented medical controversies of the early republic.

In the 19th century, yellow fever continued to strike American and Caribbean port cities with cyclical ferocity. The Memphis epidemic of 1878 killed over five thousand people and emptied the city, driving sanitary reform movements across the American South. Sanitarians of this era remained largely committed to filth-and-miasma theories, though they inadvertently reduced mosquito breeding grounds through their campaigns to drain standing water and clean streets.

The intellectual turning point came at the very end of the 19th century. Cuban physician Carlos Juan Finlay had proposed as early as 1881 that the Aedes aegypti mosquito served as the vector transmitting yellow fever between human hosts, a hypothesis the medical establishment initially dismissed. Following the Spanish-American War, the U.S. Army Yellow Fever Commission, led by Major Walter Reed, conducted a series of controlled experiments in Cuba between 1900 and 1901. Working alongside James Carroll, Aristides Agramonte, and Jesse Lazear — the latter of whom died after an infected mosquito bite during the research — the commission produced evidence confirming Finlay's mosquito transmission theory. Lazear's death was among the most mourned sacrifices in the history of American medical research.

William Crawford Gorgas subsequently applied mosquito-eradication campaigns in Havana and later in the Panama Canal Zone, achieving dramatic reductions in yellow fever incidence and demonstrating that vector control could suppress epidemic disease on a large scale. The work of the Reed commission and Gorgas transformed tropical medicine and gave epidemiologists their first clear model of arthropod-borne viral disease transmission.
